To complete the registration process, additional documents are required*:
Birth certificate
Social Security Card
Immunization Certificate (Form 3231)
Income Proof (2021 Tax Return or W2)
Proof of Residence (Mortgage Statement, Lease or Utility Bill)
Food Stamp Card
Medicaid/ Peach Care/ Private Insurance Cards
*Documents can be scanned and uploaded or dropped off in the JCPK Front Office.
